3. How do I know you are selecting the best caregiver for me?

Our caregivers have chosen to be live-in professionals because they enjoy establishing relationships and providing the care needed so seniors can remain in their own homes.

Home Care Solutions takes great care to match the right caregiver for each Client. Our Staffing Manager and Director of Nursing work closely with you to make a thoughtful selection. We are sensitive to your needs and work with you and your family to ensure a successful partnership. We match Clients based on their specific needs and the Home Health Aide's areas of expertise, personality, availability and the physical requirements needed to provide care.

4. What do your services include?

Flexible and tailored to meet your individual needs, our live-in Home Health Aide services include:

  • An initial assessment by a Registered Nurse
  • Medications ordered and set up every two weeks by a Nurse

Your Home Health Aide will provide the following services:

  • All elements of personal care including bathing, oral hygiene, shaving, hair, skin, foot and nail care as well as dressing assistance
  • Walking assistance and transferring from bed or chair
  • Meal preparation and dining
  • Medication reminders
  • Household services to maintain a safe and clean environment
  • Laundry
  • Exercise programs to maintain flexibility and health
  • Bowel/bladder control devices and training on proper usage
  • Transportation and escort service

In addition, our staff will provide:

  • Nurse supervisory visits and clinical assessments
  • Liaison service between the Client and their physician, ensuring the best Client Care Plan possible
  • Client and family support services
  • Assistance with discharge planning at the start and end of your case
  • A collaborative, supportive relationship with other agencies involved in care (physical therapy, Medicare skilled visits, hospice and others)
  • Registered Nurse and Home Care Solutions Managers are available 24 hours a day, every day of the year for Client care concerns or emergencies

5. How much does it cost to have live-in care?

At Home Care Solutions, our services are competitively priced. Unlike many agencies, we do not charge extra for weekends or scheduled medication set-ups. Please call our office for rates on either our single or couples service program.

6. Does insurance or Medicare cover any of the cost?

Medicare and medical insurance usually only cover home care visits to address a skilled need such as wound care, some post-operative care, establishing a diabetic monitoring program, physical therapy, occupational therapy and some medication injections. Live-in care is considered custodial care. As such, Medicare and traditional medical insurance do not cover home care.

However, most long term care insurance (LTC) policies will cover live-in care.

7. What geographic area does Home Care Solutions serve?

Home Care Solutions proudly serves the Minneapolis / St. Paul area. Our service region covers 11 counties including: Hennepin, Ramsey, Anoka, Carver, Dakota, Chisago, Isanti, Scott, Sherburne, Wright, and Washington counties.

8. Is a physician's referral needed?

No, Clients do not need a physician's referral to take advantage of our services. However, you do need to be "under the care" of a physician. This encourages continuity of care and assurance that each Client's medical needs are being met.

9. What criteria do you use before hiring your caregivers?

"We will only staff a caregiver in your home that we would feel secure having in the home of our own parents."

Home Care Solutions is committed to providing professional, qualified and reliable caregivers. Our staff is industry certified, must have prior caregiving experience and a demonstrated commitment to the well-being of our Clients. Every employee has passed a background check and screening process, allowing us to identify the best-quality caregivers in their profession. Only the most committed, reliable and compassionate candidates make it through this process. Once accepted, they are required to complete our own rigorous orientation program, where we continue to screen, evaluate and develop all candidates. Ongoing in-service training is also required. In addition, our Nurses perform bi-monthly supervisory visits and carefully monitor each Client's Care Plan. Activities of the Home Health Aide are carefully recorded and documented to meet or exceed the State of Minnesota's compliance and regulatory requirements. All of our Home Health Aides are certified, bonded and insured.

10. How many different caregivers will be helping me in my home?

We know that the best relationships are built when Clients and Home Health Aides have the opportunity to know each other. At Home Care Solutions, Clients are typically assigned two caregivers that provide live-in care. Each week, the primary Home Health Aides rotate to ensure continuity of care and the opportunity to bond in a professional and friendly manner, a bond built on the respect for personal dignity and privacy.

11. How do you monitor Quality Assurance?

Home Care Solutions employs a Quality Assurance Supervisor whose primary responsibility is to make "unannounced" home visits to the homes of our Clients to monitor daily activities of the Home Health Aides.

Home Care Solutions keeps in constant touch with our caregivers in Client homes via weekly phone calls by one of our nurses. Our check in calls discuss Client status / changes, menu planning, activities, any concerns regarding the Client’s Care Plan, etc. This regular communication is beneficial to all parties as it keeps the caregiver, clinical staff and families ‘on the same page’ and operating optimally.

Any time there is a change of caregivers on a case we overlap work time one hour to ensure that the HHA leaving the case is able to adequately report information to the new HHA (orientation period).

12. What happens if a Client needs a Care Manager to assist with complexities in their care?

Home Care Solutions believes that our team of skilled Nurses have the ability to identify situations that would benefit from professional Care Management. We can share the names of several very reputable providers of Care Management with you, which would eliminate any possibility of a conflict of interest (by Home Care Solutions providing Care Management).

13. How do I prepare for live-in Home Health Aide assistance?

We have a checklist we can mail or e-mail to you that provides more specific information on what you can do to prepare for a live-in caregiver. A separate sleeping area is required for the Home Health Aide. We are happy to discuss the available options. Caregivers will assist with meal preparation for both their Client and themselves. We ask that groceries be available for their use.

Adequate sleep is essential to enable our Home Health Aides to be awake and alert during daytime hours. Caregivers are required to have 8 hours of sleep each night, 5 of which should be uninterrupted sleep.
